Making an Appointment

Just call: 01257 232000 

  • Reception is manned at all times during opening times.

  • The receptionist will make an appointment for you with the clinician of your choice.

  • We have flexible opening times with early morning and late evening appointments available Monday to Thursday 8am-7pm and Friday 8am-4pm.

  • The receptionist will give you directions to the practice if you ask. There is on site parking to the right as you face the entrance.

 

Your First Appointment

  • When you arrive you will be greeted by the receptionist. She will ask you to fill in your details on the treatment record.  On the back of the form are details of the clinics data protection and payment policies, you are asked to sign and date this section.

  • You can be accompanied for treatment or if you wish or ask for a chaperone, please mention this to the therapist or receptionist before treatment starts.

  • Your therapist will then take you to a private treatment room for assessment and treatment. At your first session will be asked to give a history of your problem, your medication and past medical history. This is so an accurate diagnosis can be made.

  • Please ask questions at any time, it is very important to us that you feel comfortable with your treatments and home advice.

  • The diagnosis and planned treatment will then be discussed with you.

  • A short treatment will be given at this session.

  • You will also receive appropriate advice and a documented home exercise regime.

  • Referral to other clinicians e.g. podiatry or consultant, may be necessary but this will be discussed with you.

  • The therapist will advice you of the best frequency of treatment.

  • You would be advised to book the sessions before you leave with reception, especially if you want an early or late appointment, as they are very popular.

  • If you are paying for the treatment you will be expected to pay at the end of each session. 

 

Follow-up Treatments  

  • At each session your condition will be reviewed and treatment discussed, changes will most likely be made to both the treatment and home regime as you improve.

  • You will be given new documented exercises as necessary.

  • Dependent on your condition you may be treated in the treatment rooms and/or the gym.

 

Discharge

  • At the last session when you are ready to be discharged you will be given information to maintain you improvement.

  • We audit our work and therefore before you leave the receptionist will ask you to fill in a simple tick box evaluation of the outcome of your treatment.

  • It is our policy to confidentially write to your GP and/or referring consultant at the end of your care to update them on your progress and the outcome of your treatment. Copies of GP/consultant discharge letters can be made available on request.  
 

Questionnaires & Complaints

  • We send out questionnaires on a random basis as part of our audit, we enclose a stamped address envelope, so would be very grateful if you return it.

  • These are very helpful with continually improving the service.

  • The Chorley Clinic has a complaints procedure, please contact Elizabeth Eaves, clinic owner if you have any problems.
